At Hopewell Family Care, we provide access to your health records via Patient Fusion. Using this tool, you can see your lab results, care plans, upcoming appointments, and more.
Following your initial appointment, you will receive an email invitation to your patient portal. Click on the orange box labeled 'Access your records.' From there you will be instructed to set up your account. For detailed instructions on how to register your Patient Fusion account, click here. After the initial setup, you can access your portal by logging in from the Patient Fusion homepage. If you are an established patient and do not already have access to your patient portal, you can call our office for an email invitation, or you will be invited following your next appointment.
If you have children, you can view their patient portal using the same email address as your own. Upon request or following their appointment, you will receive an email invitation to their patient portal. Click on the orange box labeled 'Access your records." From there you will be instructed to link their account to an existing account.
