New Patient Inquiry

As of 1/1/2026, we are now accepting additions to our New Patient waitlist for patients of any age.

Click here to be added to our waitlist. Please fill out the information completely, including ID and insurance card. You will need a new form for each family member.

Please note:

• We are not taking any new patients with Medicare or TennCare. Patients with these insurances are required to disclose their full insurance status and cannot be self pay with our office.

• If you receive a confirmation email after submitting the survey, you have been successfully added to our waitlist—there is no need to follow up to confirm. You will receive a second email once your patient chart is complete with instructions to schedule your initial appointment.

• We’re excited to announce that as of November 4, 2025, we’re accepting reservations for future newborn appointments for babies that have not been born yet! Babies that have already been born, should seek care at another clinic until we are able to open up our new patient waitlist. Click here to learn more and to fill out the form to reserve your spot.

👉 Please do not wait for an evaluation with us if you have an urgent health concern. We encourage you to seek immediate care through your primary care provider, urgent care, or emergency services as needed.
